GENERAL EXPLANATIONS,

Personal data owners (“ Applicant ”), defined as the data subject in the Law on Protection of Personal Data No. 6698 (“ Law” ), have been granted the right to make certain requests regarding the processing of their personal data in Article 11 of the Law .

Pursuant to the first paragraph of Article 13 of the Law ; “ENTO KULAK NOSE BOĞAZ ÖZEL SAĞLIK HİZMETLERİ TİCARET A.Ş.”  (hereinafter briefly  referred to as “ENTOKBB”  ), applications regarding these rights must be submitted to us in writing or by other methods determined by the Personal Data Protection Board (” Board “).

In this context, applications to be made to our Institution in “ written ” form, by printing out this form;

• With the personal application of the Applicant,
• through a notary,
• It can be forwarded to us by being signed by the Applicant with the “secure electronic signature” defined in the Electronic Signature Law No. 5070 and sent to the Registered Electronic Mail (“KEP”) address of our Institution.

Below is information about the written application regarding how the written applications will be delivered to us.

In addition, after the announcement of other methods to be determined by the Board , it will be announced how the applications will be received through these methods. Your applications submitted to us are answered within 30 (thirty) days from the date of receipt of your request, in accordance with the second paragraph of Article 13 of the Law , depending on the nature of the request. Our answers are delivered to you in writing or electronically in accordance with the provisions of Article 13 of the relevant Law .

C. REQUESTS OF THE DATA SUBJECT

As a data owner, please tick the relevant box(s) in the list below for the situation(s) you would like to be informed about within the scope of Articles 11 and 13 of the Law .

In the applications to be made by third parties on behalf of the data owner, a copy of the power of attorney approved by the notary public together with this Application Form, and a copy of the documents proving the custody/guardianship relationship must be sent to us, in the applications to be made on behalf of the children under custody/guardianship.

In order to ensure the security of your personal data , “ ENTO KBB” will be able to contact you within seven (7) days from the date of receipt of your application for information, in order to confirm that you are the data owner, and may request some information and documents from you in this regard. In this context, the information and documents you have provided to us will be destroyed immediately after confirmation of your data ownership.

In case the requested information and documents are missing, the information and documents will have to be completed and forwarded to us upon our request. Until the information and documents are fully transmitted to us, the 30 (thirty) day period specified in article 13/2 of the Law regarding the conclusion of the request will be suspended.

D. CONCLUSION OF DATA SUBJECT’S REQUEST

According to its nature, your request will be answered as soon as possible and within 30 (thirty) days at the latest following the date it reaches us in accordance with the Law . Our answers and evaluations will be communicated to you in writing or electronically, in accordance with Article 13 of the Law , according to your choice stated in this Application Form . If you have a priority preference for the application result to be sent by post, e-mail or fax, please indicate below:

Your requests will be concluded free of charge by “ ENTO KBB” , and if the response process incurs additional costs, a fee may be charged in the amounts determined within the framework of the relevant legislation.

E. DATA SUBJECT DECLARATION

This application form has been prepared in order to determine your relationship with ENTOKBB   and to fully determine your personal data, if any , processed by ENTOKBB  , so that your relevant application can be answered correctly and in a legal time. Our Institution reserves the right to request additional documents and information (copy of identity card or driver’s license, etc.) for identification and authorization determination, in order to eliminate legal risks that may arise from illegal and unfair data sharing and especially to ensure the security of your personal data. In the event that the information regarding your requests you submit within the scope of the form is not correct and up-to-date or an unauthorized application is made, ENTOKBB, does not accept any liability for such wrong information or unauthorized application requests.
